Instructions for Athletic Clearance
1. Go to www.nfhslearn.com and watch the 3 student course videos: Sudden Cardiac Arrest, Concussion in Sports, and Heat Illness. Either print out or take a screenshot of the 3 completion certificates.
2. Go to athleticclearance.fhsaahome.org and begin your registration.
**Check every sport you might be interested in.
**Complete all sections, including EL3 Parent Signature Section.
3. Once you reach the Files section, you must upload the documents listed under Required Paperwork below
Required Paperwork
- Proof of Medical Insurance
Submit to Athletic Secretary
- Original Birth Certificate or Passport: for required proof of age (CLHS will make a copy)
- GA4 Form: required if you have ever attended another High School
- If you are a Non-Traditional student: Homeschool/ Virtual School, please contact the CLHS Athletics Department as there is additional paperwork required.
- If you are an International or Foreign Born student: please contact the CLHS Athletics Department for a list of required documents.
- If you attended 8th grade outside of Lee County, you will need your 8th grade Transcript/Final Report Card.
GPA: must be at least a 2.00 (State GPA)

Information on Sporting Events
Cypress Lake High School uses GoFan for all sporting events. Tickets for upcoming games and matches can be purchased on CLHS's GoFan page. Be sure to not redeem your tickets until you are with a ticket validator. We do not accept cash for tickets nor do we accept screenshots of prior ticket purchases.
In order to increase the safety of our students, staff and guests at athletic and arts events, the School District of Lee County does not allow backpacks and other large bags into stadiums, gymnasiums and theaters.
For the safety of our athletes, performers, employees and spectators, we ask for your patience as you enter school games and events.
Prohibited items include, but are not limited to:
- Book bags, backpacks, duffel bags, or large purses
- Any item that could be used as a missile
- Cans/Bottles
- Coolers
- Fireworks
- Horns/Lasers
Any type of weapon on school property is already prohibited under board policy. All guests attending events may be subject to a search. These changes implement policies similar to what the NCAA, professional sports teams, large arenas and theaters require of their guests.
